Reducing harm at the transfer of care following hospital discharge is one of the three main elements of the World Health Organization’s (WHO) Global Patient Safety. This service is recommended for everyone following discharge from the hospital because the patient is being moved from one care setting to another, so medicine reconciliation is very important.
